What is Breast Reduction Surgery and who can get it done?

Loose, heavy and full breast structure which shows the patient more overweight is not only an aesthetic problem but can also become a health issue for the patient as it can cause neck and back pains, stooped shoulders, rashes under the folds of the breasts, bad smells and infections with the passage of time distancing the patient way from social life. Finding suitable clothes also becomes next to impossible.  The patient shies away from society and becomes withdrawn.  Physical movements also become restricted. The permanent and definitive solution for all these problems is a breast reduction operation.

The purpose of this surgery is to reduce the size, weight and volume of the breast and to give them an uplift, fullness and more youthful appearance whilst protecting the milk ducts, milk glands and nipple sensation to the maximum.   During this procedure it is also possible to reduce the areola (the brown circle of the nipple) to give it a more a smaller and more aesthetic appearance. In short, the purpose of breast reduction surgery is achieve a smaller, fuller, better shaped uplifted breasts which are compatible with the body structure of the patient.

What is the earliest age that breast reduction procedure can be done? 

With the effects of the oestrogen hormones the breast develops during adolescence and grows according to genetic disposition, and weigh gain. This development continues until the ages of 17-18 and sometimes carries on up until the age of 20.  For this reason, the breast development must complete itself before surgery should be considered.

Breast reduction consultation:

The consultation is carried out in three stages as follows:

     Stage1: Gathering information

  • Listening to the patients concerns and expectations
  • Full and extensive medical details, including if they are a smoker or not, if  they have breastfed before, if there was breast milk flow, any existence of masses felt in the breast, hormone status, pregnancy expectations are asked and noted.

Stage 2: Physical examination

  • Examining the breast for any masses.
  • Examining breast skin quality
  • Taking distance measurements of each nipple
  • Assessing symmetry statues
  • Evaluating the existent breast sitting on the body

Stage 3: Operation planning

  • Full consideration of the above and the possibility of the patient breastfeeding in future are taken into account in operation planning.

Choosing the breast reduction technique:

The criteria of breast reduction operation techniques are based on the age of the patient, expectations of pregnancies, nipple droopiness and breast volume. The latest technique in breast reduction surgery is the vertical scar technique. This technique is highly suitable for young patients with medium sized breasts, medium level of breast sagginess and who has future pregnancy expectations. This procedure technique offers the least scarring and maximum protection of milk ducts and mammary glands. The scarring of this technique may be around the areola and as a vertical line going under the fold of the breast in a J or a T shape depending on the removal of the breast skin excess.

As with all other scarring the scarring is hidden in underclothes and fades over time, taking the colour of normal skin and becoming almost invisible.

In extremely large breasts where there is a high level of sagginess and in advanced age patients the incisions are made in an upside down T shape. These incisions surround the areola, extend vertically down and follow the natural folded line of the breast horizontally. The excess breast tissue, fat tissue and the skin are removed and the nipple with the areola are moved up in the shape of a flap and are paled to their new positions through a tissue. Then as the breast tissue on both sides are joined around the areola and the middle lines the new breast lines are formed. This vertical scar technique is one of the older techniques and is still used safely in favourable patients.

There is also a free nipple technique which is one of the oldest techniques around. This technique is suitable for patients with what is termed as “gigantic breasts” (gigantomachia) and in advanced age groups who is no longer associated with breastfeeding. The scarring of this operation is the same as the upside down T technique and the procedure involves complete removal of the nipple environment and stitching it to its new position after the formation of the new breasts.

Can patients with breast reduction surgeries breastfeed?

A large majority of women are able to breast feed their babies after having a breast reduction surgery. The ratio for this is 80 to 85 % with the latest available techniques and meticulous surgery.

How is a breast reduction surgery conducted?

  • The operation is performed under general anaesthesia and in a hospital environment
  • The duration of the operation is 3- 3.5 hours.
  • Anaesthetic and adrenalin injections are given to incision areas of both breasts before the start of the procedure to reduce post op pain and bleeding.
  • The excess skin and breast tissue is removed in accordance to the pre op drawings, the nipple is moved to a pre-determined position and the incisions are closed in a manner which uplifts and gives the breasts a fuller and youthful appearance.
  • A draining appliance is placed
  • Special bands called “strips” are put on the incisions to speed the healing process and to protect the cuts finalising the operation.(As self-absorbing stitching is applied, the patients do not go through the added anxiety of stitch removal procedures)
  • As a final stage of the procedure a specially designed bra corset is placed on the patient.

Is it necessary to place a draining appliance during the surgery?

Based on my experience I find it safer and healthier to place a draining appliance during breast reduction surgeries. Despite the fact the bleeding may have been stopped during the surgery with strict bleeding control the patients may experience nausea and vomiting problems for a period of time due to the effects of the anaesthesia and bleeding in stopped areas may occur again with the internal pressure of such problems (i.e. retching).

Although placement of a drainage adds to the cost of the procedure it is my personal preference to place a drain for the rapid and smooth recovery of the patient. I use a silicone based drain which is completely compatible with the body and which does not cause any additional pain.

What will I need to do before my operation?

  • Stop smoking at least 15 days before your operation schedule date and take lots of walks in fresh air after you stop smoking.
  • Herbal teas, ginseng and ginga biloba vitamins must all be stopped 1 week before the surgery schedule date.
  • You will be required to bring with you front fastened pyjamas for easier post op dressing care accessibility.

What can I expect after my surgery?

  • You will need to stay one night in the hospital
  • There is very little post op pain and can eaily be controlled with basic painkillers if required.
  • The drain is removed on the 2nd or the 3rd day after the surgery and you can have a bath the day after its removal.
  • The first dressing is done when the drain is removed. All subsequent dressing care can be done at home by the patient. The best wound care is bathing.
  • Swelling will occur 4- 5 days after the breast reduction surgery and will go away after a period of time.
  • You will be requested to sleep at a 45 degree angle for 3-4 days after your operation to assist in the prevention and reducing any swelling development.
  • In the early stages of post op the breasts will be appear to more upright and appear to be more up in their locality however they will shift slightly to fill in the lower parts of the breasts within 1-3 months.
  • Lifting weights and putting force onto the arms will need to be avoided for a period of 3-4 weeks after your op.
  • Patients can go back to their social activities and jobs after a week on the condition that their jobs are not strenuous.
  • Smoking will need to be avoided until the operation wounds are completely healed.
  • Post op swelling may cause some degree of nipple and breast skin sensation loss. This usually disappears within 6 weeks. In some patients this loss of sensation can last for a year or more and very rarely is permanent.
  • Post op scarring are designed to be hidden within the laundry and will fade during the course of time. Some helpful methods and jells will be provided to accelerate the fading process.

Lastly:

Breast reduction surgeries are one of the most popular operations in aesthetic surgery due to its minimal pain, rapid recovery duration and immediate results. You will be free of the physical discomfort caused by large and droopy breasts, your body will be more proportionate and you will be able to get better fitting suitable clothes. Those who know you will even remark that “you look younger” and that “you have lost weight”.
